DescriptionToronto Central Community Care Access Centre (CCAC) delivers home and community health care and connects people to other services in our community. We work with seniors, adults, children and their families to help determine and provide the right care and health supports to keep them at home, where they are happiest, for as long as possible.Toronto Central CCAC's services are available to over 1.5 million residents in the Toronto Central LHIN geography, resourced by our three regional offices. Approximately 46,000 Torontonians of all ages and cultural backgrounds benefit from home care services delivered by Toronto Central CCAC every year.We serve an area with the highest rates of low income and single parent families, as well as a diverse urban population. We work with 26 hospitals, 150 community-based case coordinators, 42 Long-Term Care Homes and 13 Community Health Centers. We also work with 35 health service provider organizations to deliver a range of in-home and in-school services to our clients (e.g. nursing, occupational therapy, physiotherapy, speech language pathology).In any given month Toronto Central CCAC supports:· More than 19,000 people of all ages, cultures and backgrounds· 1,700 children getting support at their schools· 400 adults receiving rehabilitation services· 23,000 information and referral inquiries· The transition to a long-term care home for 240 clients· 600 individuals to die at home with dignity· Saving 1,000s of hospital days by transitioning 7,000 clients home for care.Over 500 people work for the Toronto Central CCAC at three community offices and at 26 hospital sites in order to better serve our clients. Our Care Coordinators are regulated health professionals, with expertise in nursing, social work, occupational therapy, physiotherapy or speech therapy, who work directly with people being cared for in their homes, and all across the health care system as well.
